同意書 • Waiver

同意書は、会場に入る全員が記入•署名しなければなりません。たとえ競技に参加しなくても、会場に入る人全員が署名してください。18歳以下は保護者が署名してください。

1枚の同意書で大人2人、子供4人までが記入•署名出来ます。DocuSignの一番最初の画面で大人2人の記入をすると、2人目の分の同意書は記入されたメールアドレスに送られますので、2人目の大人はそのメールから署名をしてください。

18歳以下の子供は大人のどちらかの同意書に記入してください。大人の記入箇所の「Relationship（続柄）」の項目を記入しないと子供の名前が入力出来ませんのでご注意ください。

Everyone entering the venue must fill out and sign the waiver. Even if you are not participating in the races and games, everyone entering the venue must sign. Those under 18 must have a guardian sign for them.

One waiver can be filled out and signed by up to two adults and four children. On the initial screen of DocuSign, when entries are made for two adults, the waiver for the second person will be sent to the email address provided. The second adult should sign via that email. 

Children under 18 years of age must have a waiver signed by one of their parents. Please note that if the 'Relationship' section under the adult's part is not filled in, you will not be able to enter the child's name.

会場について • Notes on the Arena

• 飲食のために敷物をお持ち下さい。

• アリーナは人工芝のため飲食に制限があります。飲み物は水か、透明で甘味料が入っていない飲み物のみです。コーヒーは持ち込めません。また、ひまわりの種は持ち込み厳禁です。

• 飲食は必ず敷物の上で行い、敷物に落ちた食べかすやゴミは人工芝の上で払わず、そのまま持ち帰ってください。

• ストローラー、車椅子、折りたたみのローンチェアが持ち込めます。

• 運動会中の各生徒の責任は各家庭にあります。トイレの使用などは必ず保護者が付き添ってください。

• Foothills Park & Recreation全ての施設内への銃火器の持ち込みは、オープンキャリー、コンシールドキャリー共に禁止されています。

• Bring a blanket to sit, eat, and drink on.

• Due to the artificial turf in the arena, the consumption of food and drinks is restricted. Only water and non-sweetened, clear drinks are permitted. Coffee and sunflower seeds of any kind are not allowed inside the arena.

• You must consume food and drinks, other than water, on a blanket, and take any crumbs with you. Do not shake your blanket out onto the turf.

• You can bring strollers, wheelchairs, and folding lawn chairs to the arena.

• Each family is responsible for their child(ren) during the field day. Please ensure to accompany your child(ren) to the bathroom.

• The open and concealed carrying of firearms is PROHIBITED at all facilities, parks, trails, and golf courses managed by Foothills Park & Recreation.
